E-M:/ waste plant raw sewage public beach

Manhole covers blew off and raw sewage - toilet paper and all spewed into a creek that went 2 miles down  to a public Lake Erie beach over the 4th of July weekend.
The NPDES permit has no reporting requirements - the City never told anyone - the state park was never notified and reports are that the ecoli count was over 10,000.
The wastewater line is a new one that was supposed to help with failing septic systems. Instead it serves as capacity for excess flow for infiltration into the old part of the City and in heavy rainfalls discharges into the creek that goes to the beach.
It seems there are no consequences for this...  OEPA says no reporting requirements -
The NPDES permit is up for review - suggestions..
